The following criteria must be met:

• Your affection level with Ellen is lower than 30 points
• You are in Spring of Year 2

Ellen will appear to be dozing in her chair outside the bakery. You can avoid her death scene if you don't interact with her through Spring. If you see the event, you will lose 20 happiness points, Ellen will disappear forever, and dialogue about Ellen's death will be triggered in the villagers.
